Fill Out Your Forms Ahead of Time

We understand that your time is precious; therefore Center for Psychological Services has made all of our adult patient forms as well as our consent form available electronically to complete at your convenience. These will be sent to you when you call to schedule your appointment.

Adolescent specific forms may be downloaded and brought to the office or sent in via email to support@cfpsmn.com.

Please be aware that the face-to-face portion of your appointment with a therapist can only take place after you've completed all of the necessary paperwork. If you can't complete the paperwork before your appointment, please arrive at least 30 minutes early to complete it at our office.

Regarding paperwork with children and adolescents: Consent forms need to be completed and signed by a parent or guardian for all minors. In circumstances with a joint legal custody arrangement, BOTH parents need to sign consent forms before therapy can begin.

Regarding releases of information: In order for the releases to be valid, they must be made out to your specific therapist, not to Center for Psychological Services. All forms must be legibly signed and dated.
